• Notice of Information Practices (2 Pages)
• Forms Summary Agreement (2 Page)

Insurance companies require physical or occupational therapy service to be referred or “ordered” by one of the following; your medical doctor, chiropractic doctor, podiatrist, naturopath, osteopath or Dentist.

3. If you have printed and filled out all appropriate forms: please plan your trip to your scheduled appointment time to arrive 15 minutes early
